If they are anything like the set I brough home from the 'stan, the shirt sizing matters, but only the length matters on the pants. I.e., they are about 5' in diameter at the waist, and you use a woven belt to cinch them in.
I wear size small t-shirts, and mine was a size 37 for the shirt.
Generally speaking, they end up being loose enough that you can wear a ful set of 'regular' clothing beneath them. For example, I could wear a full set of ACUs and a level IIIa soft armr vest under mine, as well as inserting SAPI plates into the pockets of the man-shirt and you couldn't really tell they were there once I put on the matching Afghan vest.
